The information provided will be held on this form until the baptism takes place.
Information provided will be held in Baptismal register as permanent record: – When baptised. Child’s name; son or daughter. Parents’ Christian and surname(s). Address. Date of Birth and Minister who baptised.
We use a photograph of your child if you send us one on a PowerPoint during the baptismal service and on a limited number of printed sheets. The PowerPoint and any record of the child’s photo sent by email is deleted following the baptism and any service sheets left in the church after the service will be shredded that day.

Optional GDPR Consent agreement for us to hold information after the baptism for three years (Please mark ‘YES’ to opt in):
